About Filling Out the Church Planter Candidate Assessment Form

• Contact Dr. John Hirsch before you begin to fill out this application.
• When you open the Application Form (pdf) simply click on the form next to or below the question or catagory to fill in your responses. Should you choose to use the form feature be prepared to complete the entire form in one sitting. Once you have completed the form you may email the completed form to Dr. Hirsch by clicking the email icon on the tool bar. Also, print a copy for your own files. (You will not be able to save an incomplete application to your computer for the purpose of completing at a later date.)
